Instructions For Specimen Collection and Shipping
*Please read all instructions carefully before beginning
Step One: Check Your Kit
Included in your NutriChem Oxymark Test Kit:
*3 blood collection tubes (1 tiger top & 2 purple top)
*3 green top urine collection tubes (1 with circle on bottom to fill first)
*1 urine transfer pipette
*2 ice packs (to be placed on the urine specimen ONLY. Do not place the ice pack on the blood tubes)
*1 urine collection tube (to be discarded after transfer to green top tubes)
*1 document pouch containing an instruction sheet with health questionnaire (lab requisition is on questionnaire)
*1 absorbent pad to lay over tubes with Styrofoam lid placed on top
*1 piece red tape to seal around edge of Styrofoam box to hold lid closed to bottom tray
*1 zip lock bag for taped Styrofoam box to be placed inside
*1 Fed Ex diagnostic pre-paid shipping envelope
*1 white cardboard box
Step Two: Documentation
Each kit includes a HEALTH QUESTIONNAIRE, which also contains a LAB REQUISITION. Please ensure both of these documents are complete. Analysis of your test kit may be delayed if documentation is incomplete.
LAB REQUISITION: Have your Doctor fill in the requisition section found on the front cover on the health questionnaire. Please ensure that you have selected which testing panels you are having performed.
HEALTH QUESTIONNAIRE: The patient fills in the health questionnaire. The health questionnaire ensures that components of a patient's diet, their symptoms and medications are taken into consideration when evaluating a patients report and designing a custom supplement. This questionnaire is a very important tool, so please ensure that it is completely filled out..
Prior to Specimen Collection
NOTE: It is very important to read all of this section before beginning collection. If you have any questions please contact us. Urine specimen and blood specimen should be collected within 24 HOURS of each other. If not possible, collect urine specimen first and freeze until blood specimen is collected. If your test panels are for blood only or urine only please follow the appropriate instructions for each area.
* Vials contain preservatives-DO NOT RINSE.
* DO NOT discontinue nutritional supplements and/or medications prior to this specimen collection. Make sure you indicate what supplements and medications you are currently taking on the Health Questionnaire and this will be taken into account when deciphering your test results.
* Grapes and grape products (juice, wine) contain tartarate. False elevations of tartarate can occur if such items are part of your recent diet. Elevations of tartarate, as measured in our Organic Acid panel, indicate a possible fungal overgrowth of the bowel. Please avoid grape and grape products 24 hours prior to testing.
Step Three: Urine Collection
Note: Do not collect urine during menstruation
1 Place small ice packages in freezer the night before collecting urine specimen
2 Older children and adults collect first morning urine in COLLECTION TUBE. For children and newborns, an optional Pediatric U-bag urine specimen collector can be attached before bedtime and removed the next morning. 3 Using PLASTIC PIPETTE, transfer urine into GREEN CAP URINE SPECIMEN TUBES. The tube with white powder and/or black circle at bottom of tube should be filled FIRST. Screw the cap on tightly.
4 Remaining urine and collection tube may be discarded.
5 Freeze urine specimen if not being shipped today.
Step Four: Blood Collection
1 Fasting is required prior to collecting blood specimen. (You should not eat or drink anything except water for 8-12 hours prior to blood collection.)
2 It is best to schedule the blood drawing appointment on a MONDAY THROUGH THURSDAY ONLY.
3 Take the frozen urine specimen and all collection and shipping materials (including Lab Requisition and Health Questionnaire) with you to the blood drawing site, so you can ship the blood and urine together to our lab.
4 Collect venous blood in the BLOOD COLLECTION TUBES provided.
5 Blood samples must be sent to NutriChem the same day of collection to ensure accurate results
Note: Spinning vacutainer tubes is not necessary. NutriChem will spin blood specimen tubes on arrival. Do not freeze blood specimen.
Step Five: Shipping
1 Place URINE SPECIMEN TUBES in one half of styrofoam pack and place BLOOD COLLECTION TUBES in other half
2 Secure ICE PACKS in centre space of Styrofoam pack and place ABSORBENT PAD over tubes. Cover with Styrofoam lid. Make sure that the ice packs do not come in contact with the blood specimen as this will cause the blood specimen to be unsuitable for testing.
3 Place LAB REQUISITION and HEALTH QUESTIONNARIE in DOCUMENT POUCH and place on top of Styrofoam lid.
4 Place Styrofoam pack in CLEAR ZIP LOCK BAG and seal
5 Insert into WHITE CARDBOARD BOX
6 Place inside PRE-PAID FEDERAL EXPRESS DIAGNOSTIC ENVELOPE.
7 Complete “sender” portion of Fed Ex waybill and sign at “sender’s signature” The To/Destinataire portion of the waybill already contains our address of where the kit is the be sent.
8 Phone Fed Ex or drop off at nearest Fed Ex location. Fed Ex can be reached toll free in North America at 1-800-GoFedEx.
Step Six: NutriChem Reports
Your reports will be completed in as little as 4 weeks from their arrival at NutriChem. A payment receipt will be sent to you with your report due date printed on it. NutriChem’s lab reports can be forwarded to both you and your doctor.

Step Seven: What To Do Next
* Your laboratory reports from NutriChem are intended to provide you with information, which can allow you to move forward with your health goals.
